2007 Mobile Regional Science & Engineering Fair

Each entrant must completely fill out a basic application packet consisting of an Information Form, Checklist for Adult Sponsor (1), Student Checklist (1A), Research Plan, and Approval Form (1B)

All application packets must be post-marked by
February 16th, 2007.

An abstract form must be completed by the entrant once his/her project is finished.  This form must be submitted to the Mobile Regional Science and Engineering Fair prior to displaying the project at the Fair.  The form can be submitted the day of the Fair at the registration desk.  The entrant must include an abstract with their project display.

NOTE: Additional forms AND APPROVAL PRIOR TO BEGINNING WORK BY OUR SCIENTIFIC REVIEW COMMITTEE are required for students performing ANY work involving human subjects, vertebrate animals, potentially hazardous substances or biological agents, or human and vertebrate animal tissues; or any projects that are continuations of previous projects

Additional forms required may include:

Form 1C: Registered Research Institutional/Industrial Setting Form

Form 2: Qualified Scientist Form

Form 3: Designated Supervisor Form

Form 4: Human Subjects and Informed Consent Form

Form 5: Vertebrate Animal Form (5A and 5B)

Form 6A: Potentially Hazardous Biological Agents Form

Form 6B: Human and Vertebrate Animal Tissue Form

Form 7: Continuation Projects Form

Please note: It is the responsibility of the student entrant and his/her teacher to read and understand our rules completely prior to beginning work on a project and to recognize any special approval and/or form requirements..

If you have any questions regarding the approval and documentation requirements of your specific research project, please contact Ms. Dusty Layton (dlayton@usouthal.edu), Director of Research Compliance & Assurance, USA College of Medicine, at 251-460-6625 prior to beginning any work on your project. These types of approvals require significant time, so please contact Ms. Layton as early as possible. Because of the time constraints involved, we can not guarantee approval of any projects involving human subjects, vertebrate animals, potentially hazardous substances or biological agents, or human and vertebrate animal tissues

Failure to follow proper procedures will result in disqualification
